What is Melanoma Hope Network (MHN)?
MHN is a 501-C, non-profit, corporation located in St. Louis, Missouri. Our vision is to bring information and resources to patients and their families who are dealing with melanoma. We also would like to support researchers and clinicians who are battling this life threatening disease.
Trial Finder / Treatment Options / E-mail Notification
This search engine will search through the MHN database of clinical trials and not list (or exclude) those trials which have a requirement that does not match the profile you have entered. Although the MHN database is updated each night and it is our goal to have the most complete listing of treatment options available for melanoma patients, we can not guarantee that these are the only trials/treatment options that you may qualify for nor do we guarantee that matching a trial via our search indicates that you qualify for that trial. All trial participation and qualification is solely up to the trial administrator. You agree to completely hold harmless MHN for any mis-information and lack of information of any type found while using the site, the trial finder or e-mail notification service. Use this site as one of your sources in pursuing treatment for your illness. You should always consult your physician or oncologist prior to making any decisions about your treatment. MHN also can not guarantee that the new trial e-mail notification system will absolutely notify you of all new treatments. Once again, you should use this as one source for your information and consult with your physician or oncologist for future treatment options.
What information is Collected?
In order to deliver the level and type of service you expect from us, we collect both personally identifiable information (such as contact, health and demographic information described below) and non-personally identifiable information such as “Traffic Data” (defined below) using the methods listed below. For the most part, we use the information described in this section to customize and tailor your experience on the MHN website by displaying content and delivering opportunities that we think might interest you according to your preferences.
PERSONALLY IDENTIFIABLE INFORMATION COLLECTED
Information Collected Via the Registration Process
To take advantage of the trial search tool available on the MHN site, you need to register and obtain a user name and password. You have the option of deactivating at anytime (see the deactivation section below). Our registration form requires Members to give us basic contact information (such as your name, mailing and e-mail address) and some demographic information (such as your zip code, age and gender). The registration form information requests may change from time to time. Although additional demographic information may be requested at the time of registration, only information marked “required” is necessary for registration and utilization of our services.
Information Collected Via the Member’s Use of the Site
We collect information relating to your onsite actions (e.g. what pages you visit) to enable us to provide you more timely, relevant offers and products.
We are not responsible for the links on our site and any information that might be collected on that site once leaving the MHN site.
Information Collected Via Online Surveys, Sweepstakes and Other Special Offers.
We will, from time to time, prepare and distribute surveys and collect information submitted by you in response to those surveys. Although the surveys provide a means for us to better serve you and the ability for us to further personalize your experience on the MHN website, we do not require that you provide any of this information.
Information Collection via E-mail and Direct Mail Marketing.
We engage in e-mail and direct mail notification. E-mail or direct mail address lists for these activities are compiled from information provided by you and based upon information gathered from the MHN website/
In order to receive an e-mail message that is part of a trial finder search tool and future notification of new trials, you must have chosen to receive e-mail from us. If you are not interested in receiving e-mail from us you may choose not to receive e-mails during the course of the registration process. To further ensure we have your consent, we make sure each e-mail we send you presents an additional opportunity to “opt-out” of receiving future e-mails. We never orchestrate an e-mail marketing campaign where messages are intentionally delivered to Members who have not requested e-mail from MHN. If you are not interested in receiving either e-mail or direct mail marketing, you may choose to opt out from receiving the e-mail or deactivate your Membership (see the Opt-out and Delete/Deactivate sections below).
We collect information relating to your e-mail response data to enable us to provide more timely, relevant information and products.
We do not send or support the delivery of unsolicited bulk e-mail (a.k.a. “spam”).
NON-PERSONALLY IDENTIFIABLE INFORMATION
Traffic Data Collected
When you contact our Member Services group, we may request and collect from you non-personally identifiable information such as: (i) IP address; (ii) domain server; (iii) type and version of computer operating system (Mac or Windows); (iv) type and version of web browser (Netscape, Microsoft Explorer, etc.); and (v) the Internet service provider (“ISP”) used to access the Site in order to perform troubleshooting (i.e. to help diagnose problems with our server) or administer our Site.
We may use “cookies” for the narrow purpose of facilitating our Automatic Log-in process. A “cookie” is a small packet of information that your web browser places on your computer’s hard drive. We use them to store your Automatic Log-in ID so you do not have to re-enter it each time you visit the MHN site
We may use electronic images (called “web beacons”, “single-pixel GIFs”, or “action tags”) that allow us to count users who have visited a certain Web page or opened an e-mail, conduct research on partner and Client Web sites for auditing purposes, or to access your MHN “cookie.” Using web beacons allows us to personalize your experience by tailoring advertisements, updates, and other offers to your particular interests, and measuring the effectiveness of our e-mail communications.
Information recorded through these web beacons may be used to report aggregate demographic or usage information about registered users to our a third party, or user-level usage information to our Third Party Processors. No personally identifiable information about you is shared with a Client or Third Party Processors except as otherwise set forth in this policy.
Your information will be used to personalize and enhance your MHN experience by matching the information you provide us (i.e. status of disease and previous treatment history) with better, more relevant information available through MHN. At some point during the course of your membership we may use your information for administrative purposes (i.e. to contact you in order to answer questions or provide information). From time-to-time, we may send you general e-mail updates regarding changes or improvements to MHN or our services. We may send you e-mails that solicit donations to our non-profit organization dedicated to melanoma patients and their families. We will not disclose, lease or sell any information we gather from the Site to third parties for the purpose of sending you information about their products or services or for any other reason, except as otherwise set forth in this Policy or unless you specifically indicate your willingness to allow us to do so.
If you are a Member of the Site, we may use your information from the Site to personalize and enhance your experience on the site.
We disclaim and you agree that we will not be liable to you for any damages caused by acts or events beyond our reasonable control including, without limitation, acts of God, fires, floods, acts of terrorism, wars, civil disturbances, sabotage, accidents, labor disputes, governmental actions, and failures or delays of transportation that result in our disclosure of your personally identifiable information.
Other Web sites and Your Privacy
MHN contains links to other Web sites. Please note that these other Web sites may have privacy policies that differ from those of MHN. Several of these other Web sites may share or otherwise use the personal information you provide them. We have no control over any personal information you choose to provide those Web sites.
Except as provided above, your information is shared with third parties only on an aggregate (non-personally identifiable) basis. In certain instances, however, we may provide you with the opportunity to share some of your contact information with third parties, who subject such information to their own privacy policies. For example, you may wish to receive more information directly from a third party (i.e. trial administrator) via e-mail or postal service. But it is strictly your decision to submit any form or “click” the button to reply.
Third Party Processors
We may occasionally send information about our Members to third-party processors (“Third Party Processors”) to organize, administer, process, append and/or analyze our data on our behalf. This will help us provide more relevant information to you. All such Third Party Processors are prohibited from using your personal information except to provide these services to MHN, and they are required to maintain the confidentiality of your information. We disclaim and you agree that we will not be liable to you for any damages caused by acts or events beyond the reasonable control of a Third Party Processor including, without limitation, acts of God, fires, floods, acts of terrorism, wars, civil disturbances, sabotage, accidents, labor disputes, governmental actions, and failures or delays of transportation that result in that Third Party Processors’ disclosure of your personally identifiable information.
Although we believe we exceed industry standard practices in protecting your privacy, we cannot promise that your information or private communications can remain private one hundred percent of the time in ways not otherwise described in this Policy. For example (without limiting the foregoing), we may (and you hereby authorize us to) disclose any information about you to law enforcement or other government officials as we, in our sole discretion, believe necessary or appropriate (for example, as required by law, regulation, search warrant, subpoena, administrative agency ruling or court order, etc.).
With your consent, we use your personal contact information from our registration form to send you e-mail information and contribution solicitation. Members may opt-out of receiving e-mail and from being a registered user of the site by sending an e-mail to firstname.lastname@example.org
Personal data that you have voluntarily entered electronically is delivered to servers located in the United States. By using www.www.melanomahopenetwork.org or any of our Center’s Web sites, you consent to the transfer of this data, and to our use of such data as outlined in this Policy.
We are aware of Internet users’ concerns about the privacy of their information. We have security measures in place to protect the loss, misuse, and alteration of the information under our control. Member information is stored on servers protected by firewall, system-level and database-level security inaccessible to third parties. All data we collect is stored in a secure database server under the management of www.wingspeed.com Access to our database is limited to specified MHN and WingSpeed employees and is carefully monitored.
Delete / Deactivate
MHN gives our Members the option of deactivating their information from our databases so as not to receive future communications from MHN or any other part of our service.
To do so for the Site, simply send e-mail to email@example.com , with the words “Cancel My Site Membership” as the subject line.
It generally takes less than 30 days to remove your information from a Direct Marketing List, but in certain instances it may take up to 60 days.
We reserve the right to deactivate a Member at our sole discretion if we believe that Member has used the Site, or e-mail in a way that is fraudulent or damaging to MHN, our Clients, or Third Party Processors.
Users ability to Update Information
The Site also gives Members the option of changing information previously provided to us. To do so at any time, visit www.www.melanomahopenetwork.org and click on the “my profile” link.
We request that you update your account when any of your contact information changes. We will be unable to notify you of any modifications to this Policy or changes in any of our programs or services if you fail to update such information in a timely manner.
Lost or Stolen Information
You must promptly notify us via e-mail at firstname.lastname@example.org if your password is lost, stolen or used without permission. In such an event, we will change your password and update our records.
Policy / Site Updates
We reserve the right, at any time and without notice, to add to, update, change, modify or revise this Policy, this Site or our services simply by posting such change, update or modification on the Site. Any such addition, update, change, modification or revision will be effective immediately upon posting on the Site. If MHN materially changes how we use your personal information, we will notify you via e-mail
